Description:

The Youth Outreach Worker helps youth leave the streets and assist in moving and adjusting to a safe and appropriate living arrangement by providing shelter and other services that may include meals, laundry, transportation, child care, and assistance in identifying housing, employment, public benefits, and referral to outside services. Identify and serve trafficking victims to receive eligible services by working with outreach coordinator to promote and educate awareness on trafficking program. Provide crisis intervention information to youth clients and/or their families. Assist in outreach and recruitment activities to educate youth on available homeless/runaway services. Perform street outreach to locate high-risk youth in targeted areas. Identify community resources and provide linkages to the Sanctuary. Collect and document data as required, and assist in compiling statistical data, MIS, and monthly reports. Facilitates the intake and exit process of program participants; prepare and maintain all files in a confidential manner. Log and chart youth's behavior patterns, activities, and progress towards goals in service plan. Ensure that SOS client files are complete and accurate, and contain intake, assessment, individualized service plan, documentation of services provision/disposition, and referral outcomes. Provide full supportive services to families/youth, including client needs assessment, service plan, and regular goals review and updates. Provide information, referrals and follow-up for appropriate services and resources. Identify common points of entry for homeless/runaway clients into the Sanctuary program and assist in developing outreach routes in these high transiency areas. Collaborate with all Sanctuary programs, other EOC programs, and other partner agencies to ensure positive placement of clients, particularly in the youth shelter and transitional living centers. Assist in placing youth in the shelter, alternative age-appropriate emergency housing or transitional living center by verifying homelessness, collecting necessary signatures, and assisting in the completion of intake paperwork. Maintain positive community relations with parents, other agencies, and youth family members. Facilitates workshops (e.g. independent living skills, life skills, etc.) and/or authorized activities with program participants. Provide transportation and advocacy for health, shelter, and social services. Pickup, deliver, and/or prepare and serve meals to clients. Provide supervision of clients particpating in the Drop In Center and recreational activities on site. Assist in daily program planning and lead recreational, social and academic activities. Attend staff meetings and trainings upon request. Complete any child abuse referrals, if needed, as required of all mandated reporters.

Work Schedule:

Tuesday - Saturday
2:00pm - 10:00pm (30 minute lunch)

Requirements:

H-6 DMV PRINT OUT is required to be submitted with application. H-6 out must not be older than 30 days.

Must be at least 18 years of age or older and possess at least a high School diploma or equivalent. At least sixty college units in Human Services field, experience may be substituted on a year by year basis. Experience working with youth, homeless populations, or in a social service related area preferred. Ability to work in a non-traditional Street Outreach Services setting, with evening and weekend availability. Knowledgeable and functional on computer, including word processing, spreadsheets, and web-based reporting systems. Must pass DOJ and Child Abuse Index fingerprint clearance, TB clearance, and physical examination/health screening. Valid California driver’s license and automobile insurance. CPR/First Aid Certification, preferred. Familiarity and ability to work with youth/families of all cultural, ethnic, religious and socio-economic backgrounds. Bilingual (Spanish/English) preferred.
