Greetings CenCal and WESTOP family, 

I am asking for your urgent assistance in addressing a liability issue.  

Does anyone out there in "TRiO"land have a liability permission slip
form that they use that addresses having  "NON-Program" personnel
accompany you and your students on a trip.  

Scenario:  School district/school  that a program serves wants to send a
faculty or staff person from their school on a fieldtrip  for
supervision purposes, with your program. 

If someone has something that may fit the above scenario, please send
